Qualifying Conditions

1.     Cancer (Any Form)

2.     Chronic, severe, intractable or 

           debilitating pain

3.     Epilepsy or other intractable

           seizure disorders

4.     Multiple Sclerosis (MS), muscle

           spasms or spasticity

5.     Chronic Nausea or Cyclical

           Vomiting Syndrome that

           hasn't responded to other

           forms of treatment

6.     Post-Traumatic Stress Disorder

           (PTSD)

7.     Any other condition deemed

           appropriate by the Kentucky

           Center for Cannabis
